Description for 2023/24

This is a practice-based learning module which is undertaken during specialist community nursing practice. Students will have the opportunity to learn, work towards and achieve the given proficiencies whilst reflecting on their continuing professional development achievements and needs.
It aims to create and provide learning that is responsive, flexible and relevant to current nursing, health and social needs in community settings, working with communities, families and individuals.
The module will be delivered using a blended learning approach, and will require the practitioner to achieve clinical proficiency. This will enable the practitioner to consolidate and further progress their knowledge and skills and will encourage practitioners to deliver an evidence-based service.

Aims
Through reflective analysis and evaluation of learning throughout the programme, the student will demonstrate the achievement of proficiencies as a specialist community nurse (NMC 2001)
It aims to enhance the practitioner's knowledge and skills to enable them to utilise evidenced-based practice in order to influence and improve standards of care and service delivery.

Description of Module Assessment

1: Portfolio weighted 0%
Portfolio of evidence to demonstrate achievement of competence relating to the Practice Assessment Document
The reflective portfolio will demonstrate, through assessment of performance in practice alongside relevant supporting evidence, the student's achievement of the given competencies for the Specialist Community Nursing (District Nursing) programme. This will be coordinated and supported via a tripartite approach involving the student, Practice Assessor and Academic Assessor. The portfolio will not exceed 10,000 words.

2: Reflective Analysis weighted 100%
Reflective essay detailing professional development
A synoptic essay of 4000 words which synthesises the evidence presented in the portfolio. This will facilitate student evaluation of knowledge acquisition and application to practice, enhance standards of care and, through multi-professional collaboration, contribute to service development.

3: Coursework weighted 0%
Practice Assessment Document
The Practice Assessment Document (PAD) will detail the competencies required for successful completion of this module and the overall Specialist Community Nursing (District Nursing) programme. The PAD is based on the NMC domains and standards. Progress in the achievement of these standards will be reviewed at tripartite meetings with the student, Practice Assessor and Academic Assessor. The document will also provide the facility to record clinical hours, in line with the programme requirements.
